THE PILOT'S FAILURE TO PROPERLY PLAN THE FLIGHT AND INSURE THAT THERE WAS SUFFICIENT FUEL TO COMPLETE THE FLIGHT. THE PILOT ALSO FAILED TO MONITOR THE FUEL LEVEL DURING THE FLIGHT.
