THE PILOT'S FAILURE TO ASSURE THAT AN ADEQUATE SUPPLY OF FUEL WAS AVAILABLE TO COMPLETE THE FLIGHT. A FACTOR IN THE ACCIDENT WAS THE SIPHONING OF FUEL FROM THE FUEL CAP.
