THE IMPROPER USE OF THE FUEL SELECTOR BY THE PILOT-IN-COMMAND BY SWITCHING TO AN EMPTY FUEL TANK. CONTRIBUTING TO THE ACCIDENT WAS THE PILOT'S FAILURE TO SWITCH BACK TO A TANK WITH FUEL.
