THE PILOT'S OVER ROTATION OF THE GYROCOPTER DURING THE TAKEOFF SEQUENCE WHICH RESULTED IN A LOSS OF CONTROL. CONTRIBUTING TO THE ACCIDENT WAS THE POOR MAINTENANCE PRACTICES PERFORMED ON THE MACHINE.
