THE FAILURE OF AN UNAPPROVED WELD BETWEEN THE EXHAUST SYSTEM AND MUFFLER. THE FAILED WELD RESULTED IN THE MUFFLER SEPARATING AND AN IN-FLIGHT FIRE.
