INADEQUATE PREFLIGHT PLANNING AND PREPARATION BY THE PILOT WHICH RESULTED IN IMPROPER WEIGHT AND BALANCE (AFT CG). THE AFT CG ALSO CONTRIBUTED TO THE UNCOMMANDED ALTITUDE DEVIATION AFTER TAKEOFF.
