THAT THE PILOT INADVERTENTLY STALL/SPUN THE AIRPLANE TO THE GROUND. CONTRIBUTING TO THE ACCIDENT WAS THE PILOT'S FAILURE TO ADEQUATELY REMOVE THE ICE/FROST FROM THE WINGS BEFORE STARTING THE TAKEOFF.
